HOW CAN I JOIN THE PROJECT?

1. Referral

The first step to joining the project is through our referral system. You may be referred to the project from one of the statutory agencies such as Murray Royal Hospital, Criminal Justice Department, Social Work Department, Scottish Prison Service, or by self-referral if you are experiencing exclusion or disadvantage because of circumstances such as:
• Mental health problems
• Substance misuse
• Prison experience
• Unemployment
• Homelessness
• Community Service orders

A telephone call to the project on 01738 445384 or an e-mail to info@sixcircleperth.co.uk will set up an interview with a member of staff, which is the next step in the process.

2. Initial Interview

A member of staff will try to meet with you within a week of referral. The initial meeting is informal and you will be given general information about the project and your expected role within it so that you can make up your mind as to whether you want to take part. This meeting also provides an opportunity for you to ask questions about the project and to discuss any concerns you may have. You will be able to choose the programmes which would best meet your needs, from the Independent Living Skills, Community Projects, or Health and Wellbeing programmes. You can choose one or more of the programmes depending on your particular needs. If you, and the staff member, agree that the project can meet your individual needs a starting date will be agreed. You will be allocated a key worker when you start on the project and the role of the key worker will be explained to you.

3. Induction

On the first day of attendance you will be taken through an induction programme by a member of staff and introduced to your key worker. Your key worker will be responsible for helping you to develop an individual action plan setting out both short and long term goals, and for monitoring your progress towards these. You will be asked to complete a self assessment questionnaire to identify your needs, your strengths and weaknesses, and the areas where you feel that the project can assist you to meet your needs.

4. Assessment Process

Your progress towards meeting the goals you set out in your individual action plan will be reviewed at least twice a year to ensure that your needs are being met by the project. The assessment process is also used to measure the performance of the project and you will be asked to fill out a short questionnaire in which you will have the opportunity to highlight the strengths and weaknesses of the project, as you see it, and encouraged to put forward your own ideas and suggestions for improvement and future activities.
